Vision
To support the mental and emotional well-being of youth and families-especially those in underserved communities- through trauma-informed care, culturally responsive education and safe spaces for healing. We believe that every child and family deserves the tools to thrive, the support to grow, and the village to rise. Our vision is to break generational cycles and plant seeds of resilience, hope, and lasting wellness.
Mission
Our mission is to enhance the mental health outcomes of youth and families through trauma-informed, evidence-based interventions, prevention programming, and culturally competent care. We are committed to increasing access to quality mental health services in marginalized communities, addressing intergenerational trauma, and supporting resilience through early intervention, psychoeducation, and family-centered therapeutic models.

Meet The Team
Destinie Lambert, LCSW
Destinie is a licensed clinical social worker, therapist, and founder of Hope to Whole Wellness Collective. She earned her Master of Social Work in Mental Health and Substance Use Treatment from Loyola University Chicago and holds a Bachelor’s degree from Illinois State University.
Her academic foundation, paired with over a decade of hands-on clinical and community experience, allows her to provide deeply informed, trauma-responsive care.
Destinie has worked across hospital systems, veteran services, substance use treatment programs, and community-based mental health, supporting individuals through trauma, grief, crisis, addiction recovery, and major life transitions.
Her therapeutic approach is culturally grounded, emotionally attuned, and rooted in connection—meeting clients exactly where they are, without judgment.
Hope to Whole Wellness Collective was born from Destinie’s personal and professional journey of witnessing how many people survive without ever feeling truly whole.
As a Black woman, therapist, and single mother, she understands the weight of carrying everything while putting yourself last. She created this space to offer more than therapy — a sanctuary for healing, self-restoration, and empowerment.

Milan Myers, MPH, CHES
Milan is a Certified Health Education Specialist, Behavior Consultant, and Yoga Practitioner devoted to improving the health and well-being of our most vulnerable populations through behavioral therapy and analysis, public health research, and social-emotional health education.
